RESI Boston 2018 Innovation Challenge – Check Out the Winners

On September 6th, more than 700 scientist-entrepreneurs and investors entered the exhibition hall at Boston Marriott Copley Place to participate in the RESI conference. They were greeted by 30 Innovation Challenge finalists across therapeutics, medical device, diagnostics, and digital health sectors. The 30 companies, evaluated by LSN’s expert system and scientific & commercial review team, competed directly on merits of innovation and commercial viability. We wish all our finalists the best in bringing their products forward toward commercialization, but there could only be 3 top prize winners.

First Prize: Heartlander Surgical

The HeartLander System’s new approach to debilitating ventricular (heart) conditions comprises low cost disposables and an inexpensive, reusable controller: 1. Disposable sensor array placed on the outside of the torso 2. Disposable, tethered, percutaneously delivered, epicardial walker with multimodal sensing (e.g., Doppler ultrasound for avoiding coronaries) and insertable needle for saline-enhanced radiofrequency ablation (creates small, precise lesions) or for delivering therapy into the myocardium3. Control System that integrates sensor data, maps the heart defect, actuates the walker, plans actions, and provides a visualization display screen and control interface for the physician. Our first clinical focus (NIH-funded through animal PoC) is ventricular tachycardia, but the technology platform can be used for myocardial regeneration therapies, ventricular reshaping, and biventricular pacing.

Second Prize: BrainStem Biometrics

At BrainStem Biometrics we’re pioneering a new multi-billion medical device category to measure the most important part of the brain that no other company measures. Our first target is ICU medication safety- a much better way to administer essential but powerful sedative drugs to our most vulnerable ICU patients; the old and the young. Today most patients are over-medicated roughly 60% of the time because we have no effective way to measure the level of sedation. Administering sedatives in an optimal manner is proven to reduce length of stay and direct costs by 25% and to reduce 180-day risk of mortality significantly by 10% for tens of millions of intensive care patients. The company has initial sales, 510(k), long-life patents and is raising $800K to complete outcome studies and develop reference accounts.

How the FDA’s New Approach to Digital Health Will Impact the Investment Landscape

The FDA is working hard to modernize the agency to adapt to the rise of digital health. In his speech on April 26th, FDA Head Scott Gottlieb noted that a clear FDA regulatory strategy for digital health is critical not only for maintaining patient safety, but to provide companies and investors a clear understanding of what, if any, regulatory requirements companies will need to meet before and after market entry. Additionally, the FDA is looking to modernize their own processes by adapting digital tools to help streamline pre-market review of therapeutic safety and for improved compliance with post-market study requirements.

When considering a digital health company for potential investment, investors will ask whether or not the tool will require any sort of FDA oversight. The FDA has determined that any digital health tool that does not have the ability to significantly impact patient safety does not require any regulation. This would include tools such as general wellness apps, tools to streamline doctor-patient communication or track patient care and records, as well as tools to ease patient access to medical records. Investors focused on technologies that don’t require regulatory approval will focus on companies developing these tools when looking for digital health portfolio companies.

Challenges for Digital Therapeutic Tools

The real challenge for the FDA will come in determining the regulatory requirements for digital health tools as part of medical devices, diagnostics and therapeutics. Digital health therapeutic tools, such as tools to monitor patient compliance with medication regimens or monitoring of side effects, would be reviewed in conjunction with drug development programs. Likewise, digital health tools that serve as a component of a medical device will be evaluated as part of the device, unless the FDA determines that the software component can have no effect on patient safety. However, given the difference in time and effort required to develop or alter these software components, the FDA is still determining how to ensure that the review process protects patient safety without delaying access to these tools for long evaluation periods. Since these tools are theoretically being developed side-by-side with a drug or device, they would already be part of the regulatory process and would simply be an added component to current reviews.

New Digital Health Pre-Certification Program Offers Faster Path to Market

Of highest importance to companies and investors is the Digital Health Software Pre-Certification Program that is currently being developed. Initially, this program will only apply to tools that qualify as software as a medical device (SaMD), which is defined as any digital health tool that is not part of a device with unique hardware. This includes software tools for diagnosing, determining treatment plans, imaging, monitoring etc. and any tool using AI or machine learning for these purposes. This is the class of digital health tools that can pose a higher risk for investors due to the uncertainty with regards to regulations and what evaluation will be required before they can be implemented in the clinic, despite their increasing traction among investors. Because of the drastically shorter development time required for these tools when compared with traditional medical devices, as well as the ease in releasing new iterations, the FDA is developing a new regulatory approach: pre-certifying the company as opposed to the digital health tool.

The premise to the FDA’s new approach is that by establishing the excellence of the company releasing the digital health tool, there is an assumption that any software platform the company releases to the market will meet FDA standards. Applying and receiving precertification will allow companies to release new digital health tools to market with little or no pre-market review. Instead, the FDA will rely on post-market studies to assess if any changes need to be made. The FDA has laid out five areas it will evaluate when assessing companies for precertification: the ability to demonstrate a commitment to product quality, patient safety, clinical responsibility (defined as conducting clinical evaluation of the software product), cybersecurity responsibility and maintaining a proactive culture. While this new program is still under development, investors can now evaluate companies by assessing how they plan to meet these guidelines. Once this program is in place, this method of evaluation can serve to mitigate risk when investing in companies before their product reaches the market. Once pre-certification is available, investors can use this as a differentiator when evaluating companies with software tools already on the market.

There are still a lot of unanswered questions with regards to the exact evaluation criteria, both for pre-certification, the delineation between technologies requiring no review, what information companies must provide during the expedited review process, and what data the FDA will require from post-market evaluation. The FDA is currently soliciting feedback from those involved in the industry to help the program take shape with a list of questions related to all aspects of the Pre-Certification Program. Companies and investors in digital health should watch for more updates from the FDA as these programs and regulations take shape.

 

Hot Investor Mandate: USA-Based Single Family Office Invests Broadly Across All Healthcare Sectors, with Strongest Interest in Private Biotech Companies

A single family office based in the USA with a team of particularly strong expertise in building and growing biotechnology and pharmaceutical companies invests broadly in healthcare and is open to companies of all stages. The size of investment can largely vary depending on the stage of the company. The firm has invested mainly in USA-based companies but is open to global opportunities.

The firm has a broad interest in healthcare and is most interested in private biotech companies, with a few investments in medical device companies as well. Within therapeutics, the firm’s sweet spot is from pre-clinical to late-stage II, with particular interest in assets with accelerated or de-risked regulatory pathways. The firm is interested in all indications, including oncology and immunotherapies. The firm also considers gene therapy as an area of high interest. In terms of medical devices, the firm is most interested in minimally invasive devices and is open to 510k and PMA regulatory pathways.

The firm seeks to work with experienced management teams with strong scientific expertise. Another important consideration is other investors that have participated in the financing. The firm invests in both public and private companies and will most likely seek board representation.

Hot Investor Mandate: Large China VC with Recently Launched Healthcare Fund Seeks to Invests Up to $15M in Early-Stage Therapeutics, Medical Devices, Diagnostics, and Digital Health

A venture capital firm founded in China has over $4 billion in total managed assets in the areas of healthcare, technology and internet, and clean tech. The firm has invested in more than 70 healthcare companies that span most sectors of life sciences from pharmaceuticals and medical devices to healthcare IT, diagnostics and select healthcare services.

The firm closed their healthcare fund in early 2018, focused on early-stage health care opportunities in the US with potential market opportunities in China. The firm has USA operations in both the West and East Coast. The firm intends to lead, co-lead, or co-invest in US-based private companies in biotech for therapeutics, medtech, and digital health technologies, with equity investments ranging from $5 to $15 million. The firm will not make public company investments.

The firm is open to many sectors in the life science space – biotech therapeutics, medical devices, diagnostics, and healthcare IT. The firm is most interested in breakthrough products and technologies that address a large unmet need. The firm seeks investments in 510K devices and will invest in devices in the PMA pathway depending on stage, preferably clinical or later. In therapeutics, the firm is modality-agnostic but generally seeking opportunities that are past the pre-clinical stage. The firm is also interested in enabling technologies for drug discovery and development, and other healthcare-related technologies in the “digital-health” space.

The firm is seeking to invest in companies with novel ideas and technology and experienced and passionate management. Given the firm’s slogan of “inspiring people with the wisdom to succeed with diligence,” The firm’s team is “hands-on” and typically assumes a board seat after an investment. Investments in US companies are expected to have upside opportunities in China and can leverage the firm’s China network and expertise.

Hot Investor Mandate: West-Coast Based VC Firm Invests in Early-Stage Medical Devices, Diagnostics, and Digital Health Companies

A venture capital firm founded in 2016 and with offices in the West Coast is actively investing in Seed to Series A rounds. In addition to providing equity capital, the firm will also assist companies with intellectual property. The firm is open to opportunities in North America (USA and Canada).

The firm is interested in early-stage medical technology, diagnostics, and digital health/wellness companies and is open to medical devices of all regulatory pathways including 510k and PMA. The firm strongly prefers products that have received FDA clearance but is open to working with pre-FDA products. The firm will generally not pursue opportunities in biopharma therapeutics.

The firm seeks to work with privately owned companies that are backed by an experienced management team. The firm likes to form syndicates and prefers co-investing in financing rounds, but the firm can lead rounds as well.

Hot Investor Mandate: Investment Firm with Ties to the Chinese Government Invests Up to $10M in Therapeutics and Medtech Companies with Strong China Market Potential

When investing, the firm doesn’t only provide financial capital, they also provide their global consulting services focused specifically on helping companies work across borders and in China. Currently, the firm is partnered with the Chinese Government to bring promising projects from North America to China and are focused on technologies that are at an advanced stage with patents that are willing to launch in Chinese markets or to set up factories and labs in China. The firm is however, willing to invest smaller amounts in good opportunities that have no interest/fit for the Chinese markets as well. The firm’s major focus within the life sciences is biotech and is focused on seed to series B rounds. The firm generally invests up to $10M USD for Series A deals, is flexible for series B rounds, and invests up to $1M USD for seed stage rounds.

Within the Life Sciences space, the firm is specifically focused on clinical tests, therapeutics, and other biotech technologies. The firm is also interested in medical devices focusing on large disease areas within China such as diabetes and cancer. Additionally, the firm is focused on technologies within these sectors that have a good fit with the Chinese Market or have a strong interest in working in China.

When investing, the firm looks to take a board seat. The firm provides consulting, localization services, and the ability to leverage resources in China to help the company grow alongside their direct equity investment and believe that these abilities can be best leveraged with a board seat.
